My expertise in oral and maxillofacial surgery covers jaw deformities such as cleft lip and/or palate, prognathism, and retrognathia, as well as jaw disfunction such as temporomandibular disorders and related diseases. To deal with such diseases, most important point is not only to have a thorough knowledge of functional anatomy of oral and maxillofacial region but to understand the biological principles of the regional organs and to further explore the biological rationale for surgical treatment of the diseases. My first journey is for TMJ diseases. In the past 2 decades, the biological/biochemical process and molecular mechanisms of the TMJ disorders have been drastically revealed according to the development of biochemical analysis of the synovial fluid and surgical specimen of the TMJ. Using arthroscopy and synovial fluid analysis, we indicated that mechanical stress and systemic factor such as hormones seem to be related to the pathophysiology of the TMJ. My latest journey is surgery first approach (SFA) for orthognathic surgery. SFA without presurgical orthodontic treatment shortens its treatment time approximately 6-12 months compared with using a conventional orthodontics-first approach. I hypothesized that the shortening of treatment time could be because of enhancement of bone metabolism after orthognathic surgery. We found that the regional acceleratory phenomenn (RAP) and systemic acceleratory phenomenon (SAP) occur after osteotomy, which would accelerate orthodontic tooth movement, resulting in the reduced treatment time in SFA. The journey of biological research for jaw deformities and disfunctions never ends.

Risk factors for free flap failure and personal experience in orofacial reconstruction
Most oral oncologic and reconstructive surgeons consider free flap reconstruction as a standard protocol for orofacial defects requiring tissue grafts. This presentation aimed to figure out risk factors of free flap failure and complications in the orofacial region through retrospective analysis of my cases performed at Seoul National University Dental Hospital.
Clinical data of patients who underwent microvascular free flap reconstruction surgeries from 2006 to 2021 were collected. Total of 1395 free flaps in 1292 patients were included. Each risk factor was examined by univariate analysis using chi-square and Fisher’s exact test. Factors with p-values less than 0.10 were analyzed with multivariate logistic regression models to figure out statistically significant risk factors.
In the 1395 cases performed over 16-year period, overall success rate of free flap transfer surgeries was 97.0%. Microvascular anastomosis with saphenous vein graft resulted in statistically significant increase in flap failure rate. Higher tendency for developing local complications was shown in male, and latissimus dorsi free flap groups, in radiotherapy history patients and double free flap groups. Risk of dehiscence was increased in male, and double free flap groups while risk of bleeding was increased in the preoperative chemotherapy group. Additionally, geriatric patients (over 60 years of age) were at higher risk of developing systemic complications.
With proper planning and surgical technique, free flap reconstruction is a safe and reliable treatment option for various defects of head and neck region. Attention to known risk factors and surgeon’s experience may aid in achieving successful free flap reconstruction.
Dr Lee completed his dental education at the School of Dentistry, Seoul National University in 1982 and trained at the Department of Oral and Maxillofacial Surgery, Soul National University Hospital and military field hospital in Korea till 1988.
He has written a PhD thesis in Dentistry 1992 about “Assessment of capillary blood flow and histologic study of neovascularized bone flaps using interpositional vein graft as pedicles” and was subsequently appointed as an Associate Professor at the Chonnam University Hospital(1998-1996) and Professor at Seoul National University(1996-present).
Presently Dr Lee practices as a specialist of oncology-reconstruction and director at the Oral Cancer Center, Clinical Trial Center and Department of Oral and Maxillofacial Surgery, Seoul National University, Korea. Dr Lee has served a visiting Professor for a year in the Department of Maxillofacial and Plastic Surgery, Eberhardt Karls Universitaet Tuebingen, Germany. He also served as a research Fellow at the Laboratory for the Study of Skeletal Disorders and Rehabilitation, Children's Hospital, Harvard Medical School, USA. Dr Lee is also a Director of AOCMF Fellowship Center, Honorable President of Korean Association of Oral and Maxillofacial Surgeons(KAOMS), President of Korean Academy of Dental Science(2017-2020), and Member of The National Academy of Science, Republic of Korea(2017-life time)
Dr Lee has lectured internationally, has over 450 Publications. His clinical and research interests include peripheral nerve regeneration, bioresorbable metals, micro-reconstruction of maxillofacial region.

Value of FDG-PET/MRI in peripheral nerve injury models
Introduction: The aim of this study was to evaluate the diagnostic efficacy of 18F-FDG PET/MRI in peripheral neuropathic pain models using injured rat sciatic nerves.
Method and materials: Three experiments were performed for this study. The first experiment was performed to evaluate 18F-FDG PET/MRI as a diagnostic tool in two different peripheral neuropathic pain models. Thirteen male SD rats (7weeks, 200-250g) were used in this experiment. One rat was left intact, with no surgery performed (normal), and the other twelve rats, with operation on left sciatic nerves, were evenly divided into three groups: sham surgery group (control group), crushing injury group and chronic constriction injury (CCI) group (experimental group). The second experiment was performed to evaluate of peripheral nerve injury according to the severity of damage using 18F-FDG PET/MRI. Eighteen rats were used and evenly divided into three groups in this experiment: 30 seconds crushing injury group, 2 minutes crushing injury group and 5 minutes crushing injury group. In the first and second experiments, the degree of nerve damage was measured at 3 weeks postoperatively using following assessment methods: paw withdrawal threshold values (RevWT), maximum standardized uptake values on PET/MRI images (SUVR), and histomorphologic analysis (MAxR). The third experiment was performed to assess the recovery over time after peripheral nerve injuries using different imaging modalities, PET and MRI. Six rats were used in this experiment. Crushing nerve injuries (40N, 5 minutes) were formed in the left sciatic nerves of all rats. The degree of nerve damage was measured at 1, 2, 3, 4 and 5 weeks postoperatively using three assessment methods: RevWT, SUVR and the relative signal intensity values on MR images (MRSIR). The results were quantified and statistically analyzed.
Results: In the first experiment, compared to the control group, the crushing injury group demonstrated significant differences in RevWT (p = 0.000) and SUVR (p = 0.027) and the CCI group demonstrated significant differences in RevWT (p = 0.000), SUVR (p = 0.001) and MAxR (p = 0.048). There were no significant differences between the two experimental groups for all assessments. Correlation analysis demonstrated that RevWT and SUVR were highly correlated (r = -0.710, p = 0.010), and SUVR and MAxR were highly correlated (r = 0.611, p = 0.035). However, there was no significant correlation between RevWT and MAxR (r = -0.567, p = 0.055). In the second experiment, there was no significant differences in RevWT among three groups at 3 weeks postoperatively (p = 0.401, 0.762, 0.811). However, there were significant differences between 30 seconds and 2 minutes groups, and between 30 seconds and 5 minutes groups in SUVR (p = 0.001, 0.001). There was no significant difference between 2 minutes and 5 minutes groups (p = 0.896). In the third experiment, significant gradual recoveries of all rats were observed over time in all methods. From 4 weeks after injury, there was no significant difference with before injury in RevWT and MRSIR (p = 0.5, 0.0627, respectively), but in SUVR, there was still a significant difference with before injury at 5 weeks after injury (p = 0.0007). In the linear regression analysis, the parameters of the three assessment methods significantly decreased over time (p = 0.000, all), and the explanatory power was significant in RevWT, SUVR and MRSIR (adjusted R-squared = 0.89, 0.50, 0.37, respectively).
Conclusion: 18F-FDG PET/MRI may be a valuable imaging modality to enable noninvasive, objective diagnosis of peripheral nerve injury. PET scan is expected to be a more accurate and informative diagnostic imaging modality for peripheral nerve injury than clinical neurosensory test and MRI. MRI fused with PET may help clarify the anatomic location of soft tissue structures, including the peripheral nerves.

Comprehensive management of tumors of the midface. From virtual planning to reconstruction
Aim of this lecture is to review the management of the tumors affecting the midface from a comprehensive approach. Contemporary methods of virtual planning of the resection and reconstruction as well as the transferring of the planning into the surgical field by surgical guides and or navigation will be discussed. Reconstructive methods aiming to restore the defect after oncologic resection in the midface including the orbito-cranial region may include:
• Alloplastic Implants including patient specific implants.
• Local pedicled flaps.
• Regional pedicled flaps.
• Distant pedicled flaps.
• Microvascular flaps
Our reconstructive protocol is reviewed. Indications and disadvantages of the different techniques will be discussed.

Julio Acero is the Head of the Department of Oral and Maxillofacial Surgery of the Ramon y Cajal and the Puerta de Hierro University Hospitals in Madrid, Spain, Full Professor of Surgery at the Alcala University of Madrid. He studied Medicine and Dentistry and obtained his PhD at the university in Madrid. Dr. Acero was Research Fellow at the Spanish National Institute of Health and Research Fellow of the Council of Europe in Germany. Fellow of the European Board of Oral and Maxillofacial Surgery and Fellow Ad eundem of the Royal College of Surgeons of England as well as Fellow and Examiner of the International Board for the Certification of Specialists in Oral and Maxillofacial Surgery (IBCSOMS). He graduated as Bachelor of Dental Surgery from Dhaka Dental College under the University of Dhaka in 1980. He attained advanced training from Ghent University, Belgium in 1986 and Hokkaido University, Sapporo, Japan in 1987. He completed his Ph.D from Hokkaido University, Japan in 1992 and subsequently did postdoctoral fellowship from the same university in 1993 and 2000. He joined Dhaka Dental College, Dhaka, as a Lecturer of Oral and Maxillofacial Surgery (OMS) in 1994 and promoted as Assistant Professor in 1995, Associate Professor and Head of OMS in 1999, Professor and Head of OMS in 2005 and worked there till December, 2012. Prof. Ahmed joined Sapporo Dental College as Principal in September, 2013 and worked in the position until June, 2021. Presently he is the Chairperson of the founder organization of Sapporo Dental College & Hospital - “The foundation for Health Education Service and Research”.
His main research interests include Odontogenic tumors and Cysts, diseases of Salivary glands and temporomandibular joint dysfunction. Criteria for maxillary reconstruction: from primary closure to free flap
The etiologies of maxilla defects are diverse from benign cyst, osteoradionecrosis, jaw necrosis due to drug to malignant neoplasm. Facial disfigurement, compromised speech, dysphasia, diminished overall quality of life, oro-antral fistula, nasal regurgitation and loss of vision after eyeball exenteration are the common complication after maxillectomy. Oral and maxillofacial surgeon should know how to restore the maxillary defect according to their sizes and causing diseases. There are various methods to restore the maxillary defects such as obturator, direct closure, local flap, transposition flap, buccal fat graft, collagen graft, skin graft, temporal fascia transposition, and free flap. Among these reconstruction methods, we should select the best surgical methods to restore function and esthetics.
One of the simple treatments after maxillectomy is using removable obturator. The advantages of obturator are short operation time, direct vision of the lesion after surgery, and easy cleansing.
However, the inconveniences of using removal obturators make surgeons find complete closure of the oro-antral-nasal communications. If the defect is less than 3cm, direct closure with advance flap and buccal fat is possible. Larger defect than 3cm requires free flap reconstruction. The most commonly used free flap is radial forearm free flap which has a long vascular pedicle. If the defect is confined in the maxillary bone and implants are required, free fibular flap is suitable. Large composite tissue defect could be restored with latissimus dorsi free flap.
The best surgical result come from delicate surgical technique, well-designed presurgical planning, multidisciplinary team work and thoroughly recognizing the disease entity.

The management of facial cleft tessier 0, 30 and 7
Facial cleft Tessier ) or midline facial cleft is an extremely rare entity, occurring in probably less than 1 per 100,000 births (Kawamoto and Patel, 1998). Clinical presentation including hypertelorbitism, V-shaped frontal hairline, bifid cranium, broad nasal root, bifid nose, median cleft of the upper lip, and median cleft of the premaxilla (Urata and Kawamoto, 2003).
Facial cleft Tessier 30, known as lower midline facial cleft, or median mandibular cleft, is a rare anomaly. Median cleft of the lower jaw was first reported in 1819 by Couronne.
Facial cleft Tessier 7, or tranverse facial cleft or lateral facial cleft is a very rare case. The occurrence was reported as much as 1 from 60.000 live births to 1 of 300.000 live births (Anderson, 1965).
There are some methods known for the corrective surgery of the facial cleft. The method had been vary from simple straight -line to multiple triangular flap. The serial case reports show the differences of the method that have been done to facial cleft 0,30 and 7 and the result of each cases. The most favorable method from various reported method will be shown.

Eddy Becking studied dentistry (1986) and medicine (1996) in Groningen, the Netherlands, was trained in Oral and Maxillofacial Surgery (1992) in the VU medical center in Amsterdam. His PhD thesis (1998) was on orthognathic surgery. He was appointed Fellow of the European Board of Oral and Maxillofacial Surgeons (Edinburgh,2000) and International Fellow of the American Academy of Craniomaxillofacial Surgeons (2020). Since 2001 he is working as a consultant Oral and Maxillofacial in Haarlem. He is full professor in growth and developmental disorders of the oral and maxillofacial region the Amsterdam University Medical Centre and ACTA since 2011. From 2014-2020 he was global president of the Strassbourg Osteosynthesis Research Group (S.O.R.G.). Currently he is member of the board of the Dutch Association for Oral and Maxillofacial Surgery.
His clinical and scientifical interests are facial asymmetry, orthognathic surgery and orbito-craniofacial surgery and reconstruction including computer assisted planning and navigational surgery. He authored many scientific, peer-reviewed publications and book chapters. He is a renowned international invitational lecturer on orthognathic surgery, diagnosis and management of asymmetry and orbito-craniofacial reconstruction. He lives in Heemstede Netherlands with Loes, they have 4 children.

Computer-assisted mandibular reconstructive surgery: good points and bad points
Innovations in technology has always been the primary goal of companies and software
developers to produce a system that would allow surgeons to perform surgeries effectively and
efficiently with high level of accuracy. Computed-Assisted Surgery (CAS) was developed to
improve conventional surgery and enable surgeries to plan and more or less predict the
prognosis of treatment even before the actual surgery. It can simulate the surgery virtually and
can be translated to the actual surgery through 3D printed guides, stents or models. It is
indeed a game changer in terms of performing surgery with excellent results.
But like most technology it has its disadvantages. The most common hurdles are costing,
availability and need for familiarity. When used too often, it leads to dependence of the surgeon
to such novelty. This is most evident in trainees who would have some form of deficiency in
standard clinical judgement due to less experience in conventional surgical approaches.
The topic will discuss the advantages and disadvantages of CAS in reconstructing the
mandible. The mandible which is a part of the facial skeleton that is commonly affected with
benign and malignant tumors requires a certain degree of reconstructive accuracy since it
makes up 1/3 of overall face of the patient. Improper contouring and restoration of shape and
projection can result to unsatisfactory esthetics and profound asymmetry. That is why a
comparison between CAS and conventional reconstructive will be discussed to meet the ideal
treatment for the patient based on case at hand.

Ultrasonic instruments have been used for decades and perhaps, the first well documented applications on hard tissues dissection could be traced back 20 years ago when an Italian surgeon Vercellotti who developed a set of piezo blades for dento-alveolar surgery. The advantages are quite obvious including relatively atraumatic to the adjacent soft tissues, much better tactile feed-back & vector control during bone cutting with only light to moderate compression force needed to finish the procedures.
In this presentation, you will see and learn how Piezotomes greatly help the following common scenarios in the operating theatre such as releasing inferior alveolar neurovascular bundles from proximal segments & additional lingual osteotomy or bone contour modification to alleviate severe collision between proximal / distal segments during SSRO. The bone kerf and osteotomy vectors created by BoneScalpel was found reducing risk of unfavorable fractures of SSRO, at least in my hands. It’s also handy to preserve descending palatine neurovascular bundles when surgical planning includes upwardly / backwardly maxillary impaction. You’ll feel much more confidence using Piezo to keep soft tissue pedicles intact which is mandatory to avoid avascular necrosis of bone segments following multiple maxillary or mandibular osteotomies.
By now, almost all facial bone osteotomies were done by Piezo in my practice, which constitutes a very important part of the so called “Trainee Friendly Protocol” for routine orthognathic & concomitant facial bone contouring surgery.

A classification for clinical T4b (masticator space involvement) in oral cancer by concept of compartmentalization
In this study, we discussed the value of the compartment concept applied in the discrimination of cT4b tumours, we analyzed radiographic features in CT and MRI. Secondly, a classification system cT4b in oral cancer patients was introduced, which relied on the relationship between anatomic structures and compartmental status. Thus, in our expected, fine classification of cT4b can help do accurate surgical planning before resection.

Salivary gland tumors are uncommon, accounting for only 6 to 8 percent of the head and neck tumors. However, parotid tumor represents 80% of all salivary gland tumor. Generally, tentative diagnosis is derived from clinical presentation and imaging study, but conformed by ultrasound or CT-guided core needle biopsy or even after tumor excision with pathological verification. Facial nerve protection is of paramount importance described by facial surgery, including parotidectomy, rhytidectomy or other cosmetic surgery. Nevertheless, facial re-animation is the uppermost for post-surgical patients with facial nerve integrity. Based on the reduction of iatrogenic facial nerve injury, a nerve monitoring system is highly recommended with delicate dissection and sensitive alertness. In addition to facial nerve preservation and protection, an endoscope- or robot-assisted surgery for the parotid tail tumor is also recommended owing to cosmetic consideration. In this speech, I will show how to apply a nerve monitoring system, endoscope- or robot-assisted surgery to preserve facial nerve function and improved cosmetics, in addition to sparing normal parotid tissue, shortening surgical time, and more flexible surgical methods.

The role of palliative surgery of head and neck tumors: when cure is no longer an option
Prof. André Eckardt received his dental degree (DDS) and medical degree (MD) from Hannover Medical University and is a dual-degree trained Oral and Maxillofacial Surgeon dedicated to the treatment of patients with head and neck tumors. For more than twenty years he worked as a staff member and faculty at the Department of Oral and Maxillofacial Surgery at Hannover Medical University. Since 2013 he is acting chairman of the Department of Oral and Maxillofacial Surgery at the Academic teaching hospital, Klinikum Bremerhaven. His clinical work includes management and comprehensive treatment of patients with oral and oropharyngeal cancers, jaw, salivary gland, and cutaneous (skin) cancers. He works closely with members of the multidisciplinary tumor board (MDT) at Klinikum Bremerhaven to offer optimal treatment for patients with head and neck tumors. Furthermore, his clinical interest focuses on the primary and secondary treatment of maxillofacial fractures, treatment of cleft lip and palate, and microvascular oncologic reconstruction. Currently, he holds academic appointments at Hannover Medical University and is also active in resident and medical student education. Prof. Eckardt lectures nationally and internationally and has published over 130 journal articles, textbook chapters, and abstracts. He is an active member and international faculty of the AO CMF and serves as a member of the Editorial Board of several medical journals.

Management of ameloblastoma at dhaka dental college hospital
Introduction: Ameloblastoma is one of the most common aggressive odontogenic tumor of the jaws. Treatment of ameloblastoma has been a dilemma for decades and the treatment option included enucleation to radical resection. Last few decades dredging, a conservative approach is
gaining popularity due to its advantage of preservation of jaw bone and thus improving the quality
of life of the patients. In Bangladesh, late presentation and lack of treatment facilities are creating
hindrances in providing treatment of aggressive benign tumor.
Material & methods: Dhaka Dental College & Hospital, the tertiary care center in the capital of
Bangladesh is proving treatment to the patients affected by aggressive benign as well as
malignant tumors. This tertiary level hospital receives patients from all over the country. This
prospective study was designed to see age, sex, disease type, affected jaw, mode of treatment,
the outcome of treatment. Patients were recruited from the in-patient and out-patient department
of Oral & Maxillofacial Surgery.
Results: This study has implicated the scenario of ameloblastoma at tertiary level hospital in
Bangladesh and the outcome of their management.
Conclusion: Ameloblastoma has been a burden along with the other pathology in maxillofacial
region in Bangladesh and late presentation has been a great challenge in their management. A
Conservative approach such as the dredging method has been a promising and effective
treatment option for the management of ameloblastoma in Bangladesh.
She graduated from Dhaka Dental College under Dhaka University of Dhaka in 1989. She completed DDS former IPGM&R and completed MS in Oral & Maxillofacial Surgery in 2004 from the University of Dhaka. Due to her outstanding result, she was rewarded higher training from Royal Adelaide Hospital, Australia. She got advanced training on Arthroscopic TMJ surgery from Germany.
In 1995 she joined as a lecturer at Dhaka Dental College and was promoted to Asst. Professor in 2004. She joined as Chairman at the Department of Oral & Maxillofacial Surgery, Dhaka Dental College in 2013 and currently working as Professor & Head. She has an interest in the field of Oral Pathologies, Temporo-mandibular Joint disorder, and Dento-facial deformities.
She is now the President-elect of the Bangladesh Association of Oral & Maxillofacial Surgeons (BAMOS) and will take the charge as President from 2021. She has played a great role in the successful arrangement of the Bangladesh Conference on Oral & Maxillofacial Surgery (BCOMS 2018) as the Conference Secretary.
She is an active member of the Asian Association of Oral & Maxillofacial Surgeons (AAOMS) and also a member of the International Association of Oral & Maxillofacial Surgeons (IAOMS).
Prof. Haider is a Faciomaxillary Surgeon having been educated and trained in the United Kingdom. He is a Fellow of 3 Royal Colleges of UK and also was awarded Fellowship by the Pakistan College of Physicians & Surgeons in 2010. He is the former Principal of Karachi Medical & Dental College.
He has a broad based practice over the whole range of issues involving the mouth, jaws, face and neck. He has developed expertise in surgery and reconstruction for mouth cancer, corrective surgery for facial deformities and children born with cleft lip and palate, jaw joint disturbances, facial trauma and more commonly surgical removal of wisdom teeth and placement of dental implants.
Prof. Haider is an outstanding teacher and has been involved in the training, teaching and examining of undergrads and post graduates for the last 25 years in Pakistan, UK and USA. He is directly involved in research and has been involved in over 30 original research studies.
He has been involved with charity work and conduct free surgeries for children with cancer, which is one of the areas of his interests.
On account of his service he was appointed as specialist surgeon to the Pakistan Navy by the Chief of Naval Staff in 2000, and now holds the rank of “Surgeon Captain”. He has an outstanding career as one of the most distinguished Faciomaxillary Surgeon of Pakistan and was last year awarded by the President of Pakistan, Dr. Arif Alvi at the 11th PCOMS held in the Armed Forces Institute of Dentistry, Rawalpindi in November, 2018 for his contribution to the country.

A proper collaboration between surgeons and orthodontists for a successful surgery-first orthognathic treatment
Surgery-first (SF) orthognathic treatment was introduced more than two decades ago and its interest and indication is expanding. The patients appreciate immediate improvement in facial appearance. Post-surgical orthodontic tooth movement is much easier and more physiologically favorable after surgical elimination of the skeletal disharmony.
In spite of many advantages, SF treatment is limited to a part of orthognathic surgery patients. One of the major challenges of SF orthognathic treatment is believed to be the lack of collaboration between surgeons and orthodontists.
The roles and responsibilities of orthodontists and surgeons need to be clarified for a successful surgery-first treatment. In close collaboration, an orthodontist and a surgeon can determine whether to do surgery-first or not, one-jaw or two-jaw, and extraction or not. Who constructs a surgical occlusion? Who determines jaw position? Who fabricates surgical splint? Who is responsible for the post-surgical management?
All these issues regarding the interaction between surgeons and orthodontists will be addressed in this presentation with typical clinical cases. In particular, a proper collaboration between the two specialties will be emphasized by presenting a time flowchart for a successful and efficient SF orthognathic treatment.
He received his DDS and PhD degrees from Yonsei University in Seoul, Korea. He has treated many adult patients focusing on esthetics and periodontal health and has developed many innovative clinical techniques to improve the effectiveness and efficiency of treatment to the benefit of both the patient and practitioner. Dr. Hwang is also interested in the diagnosis of dentofacial deformity patients. He has developed many innovative devices for the evaluation of facial asymmetry 2- and 3-dimensionally. As one of early adopters of cone-beam volume imaging, he has given a special emphasis on the management of surgical cases leading to a development of a specific imaging modality named CG ceph (CBCT-generated cephalograms). Regarding surgery-first orthodontics, he incorporated it into his practice in 2009. He has developed a clinical guideline for the construction of surgical occlusion and post-surgical management in surgery-first patients so that more dentofacial deformity patients can benefit from this unique approach. As the president of Korean Society of Surgery-first Orthodontics, he has given a number of lectures and provided hands-on courses on this topic. Dr. Hwang, as a faculty at the university hospital, has maintained a successful clinical practice for 25 years. He is also recognized as the former Editor-in-Chief of Korean Journal of Orthodontics (KJO) which is well known as one of the SCI Journals

Orbital trauma and fractures with orbital wall defects are common facial fractures encountered by us, oral–maxillofacial surgeons, because of the exposed position and thin bony walls of the midface. The primary goal of surgery is to restore the pre-injury 3-D (dimensional) anatomy and volume of hard tissue, and to free incarcerated or prolapsed orbital tissue from the fracture by bridging the bony defects with reconstructive implant material and restoring the maxillofacial–orbital skeleton. Numerous studies have reported orbital fracture repair with a wide variety of implant materials such as autogenous bone and artificial materials that offer various advantages and disadvantages. The ideal orbital implant material will allow conformation to individual patients’ anatomical characteristics, remain stable over time, and are radiopaque, especially for the reconstruction of relatively large and/or complex bony walls.
Recent innovations, such as computer-assisted surgical planning and intraoperative navigation, could improve the efficacy, precision, and predictability of surgical treatments of such orbital trauma reconstruction. Multi-planar CT scans, associated with 3-D reconstruction software, show variability in anatomy among individuals and help the surgeon identify a specific bone area to resect or reconstruct. The reconstruction abilities of the software could also be used to virtually display the patient's anatomy throughout the case, allowing for stereotactic navigation. Further, computerized navigation involves a virtual interface between the intraoperative position of the surgical instruments and reconstruction of patient anatomy, which is performed using CT scans. During the surgery, the navigation system controls the position of the implants or the mobilized bone and verifies the final location. Intraoperative navigation enhances the surgeon’s ability to measure the extent of resection, to identify important anatomical landmarks, and to confirm the orientation of bone grafts. Using this approach, it is possible to reduce human error, promoting greater adherence to the preoperative plan. Furthermore, intraoperative navigation could reduce the incidence of postsurgical complications due to incorrect positioning or orientation of bone grafts, plates, or fixation screws especially for relatively large orbital wall(s) boney defects as type 3 to 4 cases.
Furthermore, I would like to discuss with you on our Japanese innovative biomaterial; u-HA/PLLA composite sheet for orbital trauma reconstruction because of their bioactive, osteoconductive and bioresorbable properties.

Current update of Medication-Related Osteonecrosis of the Jaw - 2021 Position Statement
Jin-Woo Kim received his Ph.D and DDS degree in School of Dentistry from Yonsei University, Seoul, South Korea. He is a full-time professor and the head of the Department of Oral and Maxillofacial Surgery of Mok-Dong hospital, School of Medicine, at Ewha Womans University, Seoul, South Korea. He currently heads up the research center for the Intractable Osteonecrosis of the Jaw. His research interests include bone biology, osteonecrosis of the jaw, 3D printed medical devices, obstructive sleep apnea and deep learning-based diagnosis. He received the Best Young Investigator Award from the Korean Dental Association and the Korean association of Oral and Maxillofacial surgeons. His recent work on the osteonecrosis of the jaw received the West-meets-East Award from the European Calcified Tissue Society Congress. He currently conducts various researches on osteonecrosis of the jaw regarding biomarker investigation, regenerative medicines and surgical reconstruction funded by national research foundation of Korea, Ministry of Health and welfare of Korea. Recently he was elected a national member of Young Korean Academy of Science and Technology (Y-KAST).

Chaperone is a protein or an organic molecule. Chaperone is mainly involved in protein folding. When stress is applied from inner or outer, the chaperone level is increased. Chaperone helps cells to resist stress via stabilizing protein conformation or correcting protein conformation. Recently, the application of chaperone has been illuminated for treating many types of diseases. As chaperone can bind to protein, it can increase the stress on the endoplasmic reticulum. The application of chaperone can be helpful for accelerating bone regeneration. 4-Hexylresorcinol (4HR) is amphiphilic organic chemical and auto-regulator for micro-organism. As 4HR administration induces the stress on the endoplasmic reticulum, 4HR changes protein folding. The application of 4HR inhibits NF-κB signal pathway and TNF-α production. In addition, 4HR administration increases VEGF, TGF-β1, and calcification associated proteins. As a consequence, 4HR administration increases angiogenesis and bone formation in wounded area. Strong anti-inflammatory reaction and capillary regeneration in diabetic model demonstrate that 4HR can be applied on many types of surgical wound.

Dr. Balasubramanya Kumar is a reputed maxillofacial surgeon with a experience in head and neck surgery of nearly 20 years. He has a special interest in orthognathic surgery and ablative surgical oncology.
He completed his basic dentistry from Oxford Dental College, Bangalore in 1999. He then completed his masters in Oral and Maxillofacial Surgey (MDS) from Bangalore Institute of Dental Sciences, Bangalore in 2003.
He is now well known for his contribution to the fraternity as a national and international guest speaker in the field of Maxillofacial surgery.
He has completed his International board certification and also holds a CAQ (certificate of added qualification) in Oral oncology.

Dr. Mike Y.Y. Leung is a Clinical Associate Professor and Postgraduate Programme Director in Oral and Maxillofacial Surgery, Faculty of Dentistry, The University of Hong Kong. He also serves as the Assistant Dean (External Relations) of the Faculty. He is one of the Editor-in-Chief of Craniomaxillofacial Trauma Open, and Editor of several international peer reviewed journals. Internationally, he represents Asia Pacific in the AOCMF Education Commission.
He graduated BDS, MDS (OMFS) and PhD from the University of Hong Kong and started his career in oral and maxillofacial surgery in Hong Kong after graduation of BDS. He devotes to be an academic surgeon after he received the Young Lecturer’s Award of the 19th Convocation of Royal Australasian College of Dental Surgeons and the Gold Medal of the Best Original Research by Trainee, Hong Kong Academy of Medicine during his surgical training in 2008.
Dr Leung’s clinical and research interests are orthognathic surgery, management of obstructive sleep apnea and trigeminal nerve repair. He has published over 100 scientific and conference papers in the field of oral and maxillofacial surgery. Dr William Lim served as a Consultant in the National Dental Centre Singapore where he was the Head of the Oral and Craniofacial Implant Program. This involved clinical management and education of the graduate students.

Dr. Michael Miloro is Professor and Head of Oral and Maxillofacial Surgery at the University of Illinois at Chicago College of Dentistry. He attended college at the University of Rochester earning a B.S. degree in Neuroscience. He attended dental school at Tufts University in Boston, and medical school and residency training in General Surgery, and Oral and Maxillofacial Surgery at the University of Pennsylvania in Philadelphia. Dr. Miloro has held academic positions at The Ohio State University, University of Maryland, and University of Nebraska. Dr. Miloro is a Diplomate and past-Examiner for the American Board of Oral and Maxillofacial Surgery, a Faculty Fellow of the American Association of Oral and Maxillofacial Surgeons, a Fellow of the International Association of Oral and Maxillofacial Surgeons, and a Fellow of the American College of Surgeons.
Dr. Miloro has major clinical and research interests in orthognathic surgery, TMJ surgery, implant surgery, and trigeminal nerve injuries and reconstruction. Dr. Miloro has lectured nationally and internationally, has published extensively in the medical and dental literature with over 100 peer-reviewed publications. Dr. Miloro currently serves as the Section Editor of the Journal of Oral and Maxillofacial Surgery, and is Editor of three major textbooks including Peterson's Principles of Oral and Maxillofacial Surgery, Complications in Oral and Maxillofacial Surgery, and Trigeminal Nerve Injuries.
Professor Toru Nagao is a Chief Maxillofacial Surgeon and a Director of Aichi Gakuin University Dental Hospital, Japan. He practices the full scope of oral and maxillofacial surgery. His specialties are surgical oncology for oral cancer as well as management of other oral and maxillofacial diseases. Besides his clinical expertise, he has been active in the areas of early detection and prevention for oral cancer, particularly for tobacco control. He was a post-doctoral senior research fellow, WHO Collaborating Centre for Oral Cancer/Oral Medicine, King’s College, London. Dr. Toru is a Councilor of Japanese Society for Oral and Maxillofacial Surgeons, Japanese Academy of Maxillofacial Implants and an executive member of Asian Society of Oral and Maxillofacial Surgeons. Cleft nasal deformity (CND) is congenitally present in patients with cleft lip as a result of cleft itself. We refer this inherent condition as a primary CND, which usually involves the skin, nasal cartilage, mucosa, and the skeletal platform of the nose. The secondary CND is a clinical situation with a persistent nasal distortion despite of repeated surgical interventions for correction. Sometimes the CND is aggravated by iatrogenic injuries or scarring from prior surgeries. Additionally, CND is changed with the effect of patients' facial growth. Consequently, it is very difficult and challenging to correct this complex CND.
The severity of the CND is dependent of the severity of cleft. And the CND in patients with a unilateral cleft has a different pattern from the CND related with a bilateral cleft. Representative features of unilateral CND are asymmetric nose due to inferior, lateral, and posterior displacement of cleft-sided lower lateral cartilage (LLC) and septal deviation. Whereas representative bilateral CNDs involves flattened LLCs, short columella, and broad nasal tip. All of these nasal distortions are implicated in tissue dificiency around the cleft, alveolar/maxillary hypoplasia, and abnormal muscular pull on the nasal components.
Of course, it is impossible to correct the CND at single operation due to the tissue deficiencies and growth issues. So, various surgical concepts and techniques have been proposed, but there is no consensus about the most appropriate timing or the most efficient methods for cleft rhinoplasty. Status of CND and individual needs for the cleft rhinoplasty is quite different in each patient as well as in each society. Therefore, the protocol for cleft rhinoplasty can be individually tailored. In this presentation, I'd like to focus on my experiences about efficient primary cleft rhinoplasties and a definitive unilateral cleft rhinoplasty by structural component grafting using a rib cartilage.

Dr Paul J Sambrook graduated from The University of Adelaide with his BDS in 1985 and MDS (Oral and Maxillofacial Surgery) in 1990 and MBBS in 1993. He gained Fellowship to the Royal Australasian College of Dental Surgeons, Section of Oral and Maxillofacial Surgery in 1990. Paul is the Director of the Oral and Maxillofacial Surgery Unit, SA Dental Service/Royal Adelaide Hospital/The University of Adelaide, and is the group leader Oral and Maxillofacial Surgery. He is also a Visiting Consultant to a number of major public hospitals and runs a busy specialist practice in Adelaide.
Paul is the immediate past president of RACDS. He is a past Chair of the Board of Studies (OMS) from 2007 until 2013. He is a past president of the Australian and New Zealand Association of Oral and Maxillofacial Surgeons. Current position is the President of the International Board for the Certification of Specialists in Oral and Maxillofacial Surgery, and member of the IAOMS executive

Dr Seah Tian Ee obtained the Masters in Dental Surgery (Oral Maxillofacial Surgery) in 2005. He has special interests in rhinoplasty, facial aesthetics as well as orthognathic surgery and in 2009, was awarded the Health Manpower Development Plan Award to undergo training in Aesthetic Facial Surgery in UK (Fellowship in Aesthetic Facial Surgery) with Professor Velupillai Ilankovan. Dr Seah has spoken in many international and regional conferences. He also co-authored a textbook, “Local flaps in facial reconstruction” with his mentor, Professor Ilankovan as well as several chapters in oral and maxillofacial textbooks. He is now in private practice and is the director of TES Clinic for Face and Jaw. Dr Seah is also a visiting consultant to Kadang Kerbau Women and Children’s hospital, National Dental Centre Singapore, and Singapore General Hospital. He is the Immediate Past President of the Asian Association of Oral and Maxillofacial Surgeons and the Association of Oral and Maxillofacial Surgeons, Singapore.
The bone regeneration is an important challenge in the field of implantology. The placement of dental implants can be restricted by deficiency of the alveolar ridge caused by resorption of bone after extraction of a tooth which is invariably followed by significant dimensional changes in the alveolar bone dimension, leading to reduction of the height and width of the alveolar ridge and to a reduced amount of available residual bone for implant placement. Thus, preserving the dimensions of alveolar ridge is an essential factor.
The regeneration solutions aim to develop novel treatments to restore tissue function. Many surgical principles and materials can be used to fill the extraction socket, aiming to keep the shape and the size of the extracted tooth socket and facilitating in placing the dental implants. The grafting materials traditionally used include autogenous, allografts xenografts, or alloplast materials and use of autologous bone is considered the gold standard in bone regeneration.
Presently more research is been carried out to use growth factors incorporated in carriers, stimulation of the selective production of growth factors by gene therapy and use of mesenchymal stem cells (MSCs).
This presentation will discuss the presently practiced techniques in regeneration of bone in dental implantology.

Dr. Richard Su’s main research areas include clinical and translational research in oral and maxillofacial oncology, microsurgical reconstruction, and salivary gland diseases. Virtual surgical planning (VSP ) and surgical guides in OMFS.
Surgical Procedures in the Face and Jaws are a highly challenging as it involves both Function And Esthetics . Adequate and appropriate planning is obsolutely essential in the surgical outcome . Though our conventional investigations and radiographs help us in proper diagnosis and it does give minimal idea about the surgical planning and Simulation .
Virtual Surgical Planning ( VSP ) and surgical guide prepared through the digital planning had come very handy for a surgeon to plan and execute the surgical procedures like Orthognathic , TMJ surgeries etc.

A/Prof Goh Bee Tin is Head and Senior Consultant in the Department of Oral and Maxillofacial Surgery (OMS), Research Director and Deputy Director (Research and Education) at the National Dental Centre of Singapore (NDCS). Dr Goh is also Vice-Chair, Research in the SingHealth Duke-NUS Oral Health Academic Clinical Program and Director of the National Dental Research Institute Singapore. A/Prof Goh obtained her Bachelor of Dental Surgery from the National University of Singapore (NUS) in 1993. She then went on to specialize in the field of OMS, obtaining her Dental Fellowship with the Edinburgh Royal College of Surgeons in 1996 and Master of Dental Surgery from NUS in 1997. She was admitted as a Fellow of the Academy of Medicine, Singapore in 2000. In that same year, she was sent on a government scholarship for advanced training in cleft lip and palate and orthognathic surgery in Canada. In 2004, she furthered her training in craniofacial implantology in Sweden and the United Kingdom. She obtained a PhD in 2009 from Radboud University Nimegen in the Netherlands for her novel research work on modular endoprosthesis for mandibular reconstruction. She received the Osteopore Innovation Award for her work in clinical tissue engineering and the SingHealth GCEO Outstanding Clinician Researcher Award for her contributions to research at the NDCS. A/Prof Goh is also appointed Clinical Associate Professor at the NUS Dental Faculty and Adjunct Associate Professor at the Nanyang Technological University. She is active in teaching and training postgraduate students. Her research interests include bone tissue engineering and mandibular reconstruction.

The use of 3D planning and customised implants in the field of orthognathic surgery has seen increasing use and adoption. Do we really need to use these advances? The craniomaxillofacial tissue defects caused by head neck ablative surgery, deformity or severe trauma would physiologically and psychologically affect paediatric patients' life quality. However, the complexity of this regional anatomy in child (less than 18 years old) makes it a great challenge for surgeons to reconstruct some organ's function, facial contour and rehabilitate the occlusal function. With the development of microsurgery, dental implant, distract osteogenesis and digital surgery, head and neck reconstruction were achieved great progress rapidly in the last decades. Especially for paediatric patients, we did some new works in the last twenty years in our department (Dept. Oral & Maxillofacial Surgery, Peking University School & Hospital of Stomatology), we will introduce as following: 1. Small organ transplantation in head and neck area for paediatric patients- microvascular submandibular gland transfer for severe cases of keratoconjunctivitis sicca; 2. Reconstruction of soft tissue defects with different methods after head neck ablative surgery, deformity or trauma in paediatric patients. 3. Reconstruction of craniomaxillofacial bones which including mandibular restore and reconstruction, and midface and maxillary reconstruction.

He has been sponsored by 5 National Natural Science Foundation of China and more than 10 provincial/bureau projects. He published more than 140 scientific papers in national and international journals; published “Peripherial Facial Palsy” in Chinese as a co-author; also he was chapter author in Digital Dentistry editor by Rekow Dianna.
